Voluntary Dental Plan

Aetna provides a comprehensive dental plan for you and your eligible dependents. Preventive, Basic and Major services are covered as well as Orthodontics (braces) for your dependent children prior to their 20th birthday.

The dental plan pays a specified amount for covered dental expenses when you receive dental care in-network. In-network providers have agreed to provide services to participants at reduced or contracted fees. The plan will pay the same amount to a non-network provider; however, non-network providers have not agreed to the reduced fees, therefore, the remaining balance is the participant’s responsibility.

Voluntary Vision Plan

Our voluntary vision coverage is offered through Aetna. Each eligible employee may elect to participate in the voluntary vision plan and the employee pays the full cost of the coverage. In-network services are primarily copays and out-of-network benefits are based on a reimbursement schedule.

Periodic eye examinations are an important part of routine preventive healthcare. Early diagnosis and treatment are important for maintaining good vision and preventing permanent vision loss. Eye exams can detect symptoms of diseases such as diabetes, hypertension, multiple sclerosis, brain tumors, osteoporosis and rheumatoid arthritis.

Voluntary Term Life Insurance

UniqueHR also offers its employees and client companies access to voluntary supplemental insurance benefits.

Voluntary Disability Insurance

Short-Term Disability (STD) insurance will help protect your family’s financial security in the event that you become disabled. It is a paycheck replacement of sorts, while you are experiencing an illness or non-work related injury that prevents you from working as long as you meet the policy’s definition of disability. Dearborn National offers this voluntary coverage. The cost of the coverage depends upon your earnings and your desired benefit amount.

Long-Term Disability (LTD) is designed to replace a portion of your monthly salary in the event of a covered, extended period of disability due to a non-work related injury or illness. LTD integrates with and begins after STD benefits have been exhausted. To prevent over-insurance, benefit payments are reduced by deductible sources of income. The cost of the coverage depends upon your earnings, your desired benefit amount and your age.
